Name: School: Grade: What is your email address? Name of parent/guardian: What is your parent/guardian's email address? What neighborhood do you live in? Where do you see power in your everyday life (i.e. a power dynamic you would like to change, a time you've felt empowered)?
Is there one organization you would like to work with/one issue you feel particularly tied to? How many hours are you interested in doing/are you required to do? What role do you think students should play in social activism?
